About Us
Compassionate Care,
Delivered Where it Matters Most
At Key Medical Care, we believe great healthcare begins with trust, time, and teamwork — and that means meeting patients and families where they are. Whether in the comfort of a patient’s home, a skilled nursing facility, or through a seamless virtual visit, our mission is to bring personalized, compassionate medical care to older adults with complex needs.

We specialize in geriatric primary care, focusing on chronic illness management, serious illness care, and coordination with home health, hospice, and specialists. But we don’t just treat medical conditions — we care for people, and that includes their families. Every visit is an opportunity to listen, educate, and partner with loved ones to create a care plan that honors the patient’s goals, dignity, and independence.
